What to look for in a documentation audit that will cause a denial of payment for services provided by the therapist in the event of a payer review of documentation.
We will start with the evaluation and go through the necessary information needed to support the level of clinical services provided throughout the POC.
Additionally, we review some common denial reasons that are easily preventable.
